WAIVER OF LIABILITY, RELEASE, ASSUMPTION OF RISK, AND INDEMNIFICATION AGREEMENT FOR Youth conference, [Name of Participant's Legal Guardian], as the legal guardian of [Name of Participant], hereby acknowledge and agree to the terms of this Waiver of Liability, Release, Assumption of Risk, and Indemnification Agreement (the "Agreement") in favor of [Youth Conference], its officers, directors, employees, volunteers, and agents (collectively referred to as the "conference").In consideration of the opportunity to participate in the Youth Conference program (the "Program") offered by the conference, I acknowledge and agree as follows:

ACKNOWLEDGEMENT OF RISK: I acknowledge and understand that the Program involves activities that may be hazardous to the Participant, including but not limited to, sports activities, outdoor activities, transportation, and other recreational activities. I recognize that the risk of injury or harm is inherent in such activities and that the Participant's participation in the Program is voluntary.

RELEASE AND WAIVER OF LIABILITY: In consideration of the Participant being permitted to participate in the Program, I hereby release and discharge the conference from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the Participant or any property belonging to the Participant while participating in the Conference, whether caused by the negligence of the Conference, its officers, directors, employees, volunteers, or agents or otherwise.

INDEMNIFICATION: I agree to indemnify and hold harmless the conference from any loss, liability, damage, or cost, including court costs and attorney's fees, that the conference may incur due to the Participant's participation in the Program, whether caused by the negligence of the Participant, me, or otherwise.

MEDICAL TREATMENT: I authorize the Conference to seek and obtain medical treatment for the Participant in the event of an emergency, and I understand that the Conference will make reasonable efforts to notify me in such an event.

AUTHORIZATION: I authorize the Conference to use photographs and videos of the Participant taken during the Conference for promotional purposes.APPLICABLE LAW: This Agreement shall be governed by and construed in accordance with the laws of the state in which the Conference takes place.
